Nantwich care team helps Christine return home after six months in hospital
A coordinated package of support was put in place following major surgery, helping the long-standing care client rebuild her independence.

A Nantwich care provider has highlighted the recovery of one of its long-standing clients after she was able to return home following a six-month stay in hospital.
Christine, who has cerebral palsy, underwent surgery at Salford Royal Hospital to remove a brain tumour located behind her right ear.
Her condition and lengthy hospital stay meant additional support was needed before she could safely resume life at home.
Staff from SureCare Central Cheshire worked to put a care package in place, adapting the support Christine received as her needs changed following surgery.
The team had already been providing care to Christine before she went into hospital, giving staff an understanding of her routines and the assistance she required.
Following her discharge, the focus turned towards helping her regain confidence and as much independence as possible after spending half a year away from home.
The case illustrates the role home-care services can play when patients leave hospital following lengthy periods of treatment.
For people with additional care needs, returning home can involve more than simply being discharged, with support often needing to be coordinated around mobility, everyday tasks and changing levels of independence.
